Lakeland Hospice Foundation

Every year, Lakeland Hospice provides care to hundreds of terminally ill patients and their families in over 75 communities.  This would not be possible without the financial support of the families, friends, and communities we serve.

THE FOUNDATION

The Lakeland Hospice Foundation was formed in 2007 with a nest egg of endowed funds raised over the last two decades.  It was established to raise, manage, protect, and distribute funds to the Hospice program within Lakeland Hospice & Home Care, Inc.  We have made a commitment to help anyone who needs Hospice Care and because of that commitment we rely on the generosity of the families and communities we serve to realize our mission.

The Purpose of the Lakeland Hospice Foundation is to support Lakeland Hospice in their mission to provide compassionate, emotional, spiritual, and quality healthcare to each patient, promoting independence, comfort, and dignity in all stages of life’s journey.

The Lakeland Hospice Foundation will ensure that quality end-of-life care, volunteer support, grief support services, and education are available to everyone who needs it, regardless of insurance status or ability to pay.

Lakeland Hospice & Home Care, Inc. and the Lakeland Hospice Foundation are 501.c.3. nonprofit charitable organizations.  Contributions are tax deductible in accordance with the Federal Internal Revenue Service and the State of Minnesota.  Donors should contact their tax advisor about the tax deductibility of gifts.

THE NEED

Hospice Care is a benefit of Medicare, Medicaid, and most private insurance companies.  This is intended to cover all of the care provided by the Interdisciplinary Team: Medical Director, RN Case Managers, LPNs, Home Health Aides, Social Workers, Chaplains, Bereavement Coordinators, Volunteer Program Coordinator, Physical, Occupational, and Speech Therapists.  We are also responsible for 100% of the medications needed to manage the patient’s terminal illness, all of the medical supplies, oxygen equipment, and all of the durable medical equipment the patient might need, including hospital beds, shower chairs, wheelchairs and walkers, and the like.  If the patient should need IV therapy, tube feeding, lab testing, blood transfusions or radiation/chemotherapy for pain control, we are also responsible for those costs.

Typically, the reimbursement we receive from Medicare covers approximately 80% of what it actually costs to provide this level of care to our patients.  Private insurance typically reimburses even less.

Because of the financial support we receive from our donors, we are able to say “Yes” more often.  We can say “Yes, we will admit your loved one even though we know their care will be very expensive.”  “Yes, we will provide more nursing visits if you request it.”  “Yes, we will cover this expensive treatment if it will help ease your pain.”  “Yes, we will be there for you, every step of the way, whenever you need us.”

WHERE YOUR DONATION HELPS

Your gift goes directly to Lakeland Hospice to provide patient care and support for terminally ill patients and their families.  Gifts of cash, stock, or other assets or gifts in kind are all used to support patient care and services, such as the Grief Support, Volunteer Services, medical equipment, and technology updates.

Every dollar is kept here in our service area and goes directly to the care of your friends, families, and neighbors.

THE OPPORTUNITY

You can play an active role in helping your friends, neighbors, and loved ones during their time of greatest need by making a gift to Lakeland Hospice’s endowment.  Large or small, every gift counts.

There are many ways to give.  You may give gifts of cash, land, stocks, insurance, bonds, IRA’s, annuities, or some types of personal property.

There are also special ways to give your gift that can have beneficial tax and financial benefits to you.  Although each person’s situation is unique, these are some ways other Lakeland Hospice donors have given:

MEMORIAL GIFTS

The Memorial Giving Program allows donors the opportunity to remember family or friends by a memorial gift in their name.  A memorial gift honors a loved one who has passed away or may be made in honor of anyone that a donor wishes to recognize.  Gifts in memory or in honor of someone helps Lakeland Hospice to continue to provide quality care to our patients, regardless of insurance status or ability to pay.

PLANNED GIFTS

If you wish to make a donation of stock or other assets, please call the Lakeland Hospice Foundation at 218-998-1423.

WILLS:  Donors can leave a gift to Lakeland Hospice Foundation in their wills.  This can be done by leaving tangible property such as antiques, jewelry, home, or vehicle.  It can be a cash gift of a specified amount, or it can be a percentage of your estate (25% for example).

CHARITABLE REMAINDER TRUSTS:  These trusts allow the donor or other beneficiaries to receive an income.  At the end of a specified period of time, proceeds of the trust go to Lakeland Hospice Foundation.  One benefit to this type of gift includes a tax deduction in the  year the trust was formed.  This also provides the flexibility for the donor or  designated beneficiaries to receive income for life or for a period of years.  Common examples of these trusts are annuity trusts or unitrusts.

CHARITABLE LEAD TRUST: Unlike remainder trusts, lead trusts provide Lakeland Hospice Foundation with a fixed income for a specific period of time and then the balance goes to the donor’s designated beneficiaries.

LEAVING YOUR LEGACY

Whatever method you choose, and whatever size and kind of gift you make, your generosity is important to us.  You play an important role in the growth and success of Lakeland Hospice.  More importantly, you help to touch another’s life during a difficult time.
